Idler bolt is rubbing on back of large gear

Description

We are seeing a number of the v3s that come back have circular groves worn into the large gear on the back. Based on the placement, these seem to be coming from the M5x14 SHCS that mounts the idler bearing into the idler. I measured a few of the bolts we have in production and they seem to be a little longer than 14mm on average. The propensity of the idler to come out of alignment as mentioned in T4418 is contributing to this issue.

This should be resolved with the addition of a washer in the idler lever assembly (step 11 here: https://ohai.lulzbot.com/project/dual-extruder-v3/hot-end-tool-head-assembly/)
